Working on Playframework 23 - how to map checkboxes in Play Framework with Scala Forms?

Field Constructor, lang: play.
And be checked or unchecked based on a seq type mapping in the form.

I want to use this [Play-Framework] [Play 2.1.3 Scala] Dynamic lists of checkboxes with the sidebar as my main navigation
