How to setup a reverse proxy on several ports tcp udp

I have setup reverse proxy for website, and now I want to proxy my game server aswell with ports stated below, but I realy can't find anywhere how to perform this.

The service listens on both ports.
Call setup and teardown are generally performed by SIP. This information taken from UM protocols, ports, and services: Exchange 2013 Help

There are service providers that will comfortably and reliably do this for you
