PHP Mailer is giving me error 404

Here is my html for my form, action and everything else should be correct, I'm thinking its my php.

Likewise, you can follow your mail logs for specific errors.
There may be some compatibility issues but the error reporting will let us know.
Alternately, you could navigate to the file PHP file directly, and it should run.
